Despite the availability of effective lipid-lowering therapies and established treatment guidelines, many patients fail to reach recommended LDL-c targets after an acute cardiovascular event. Patients remain therefore at continued risk of recurrent cardiovascular events. Digital cardiovascular risk management (CVRM) offers a promising solution to improve adherence and outcomes through more personalized, data-driven care. The DIGITAL study implements and evaluates a digital CVRM pathway that includes home-based LDL-c monitoring and structured, nurse-led follow-up. By integrating these tools into routine care, the study aims to enable better risk stratification, earlier intervention, and improved lipid control, ultimately leading to improved clinical outcomes. Study Highlights: • Objective: To assess the effectiveness of digital CVRM in helping patients achieve LDL-c targets and reduce cardiovascular events. • Participants: 500–750 adults eligible for lipid-lowering therapy, including both primary and secondary prevention populations. • Design: Prospective, observational cohort study.
