Angina remains common—even after successful treatment for blocked coronary arteries. Studies like ISCHEMIA have shown that standard revascularization often fails to fully relieve symptoms. This is because angina isn't always caused by blockages alone—it can also stem from microvascular disease or coronary spasm.
The ILIAS OBSTRUCTIVE Trial is the first multicenter randomized study comparing standard care to a comprehensive coronary function testing (CFT)-guided strategy in patients with obstructive coronary artery disease.

- 450 patients, randomized 4:1 (CFT-guided vs standard care)
- Primary outcome: improvement in symptoms and quality of life at 6 months (Seattle Angina Questionnaire)
